NHS HEALTH CHECKS

NHS Health checks is a health check-up for adults in England aged 40-70.

It’s designed to spot early signs of stroke, kidney disease, heart disease, type 2 diabetes or dementia.

OHL are supporting Lewisham practices meet health check targets for 2023/24. 

There are two appointment types available:

1. OHL NHS Healthcheck by telephone 

2. OHL NHS Healthcheck, face to face

Eligible patients who have had all  measurements taken (e.g. hb1ac, cholesterol, height and weight) can be booked into a telephone appointment to complete a healthcheck remotely.

Eligible patients that have not had measurements taken can be booked in for a face to face appointment.
