Long-Term Condition Reviews

Our LTC Review service is designed to assist GP practices in providing housebound patients with chronic long-term conditions (LTC), an annual home visit for review.
 
The service aims to gather patient data which will support QOF reviews, increasing their confidence to self-manage and improving their health outcomes.

Our Healthcare Assistants work with GP practices to deliver this service. Facilitating proactive and coordinated care, whilst supporting Lewisham primary care with additional capacity in the community for LTC patients.

Eligibility criteria:

• Any housebound patients (including temporarily housebound).

• ​Registered with a Lewisham GP practice, with one or more long-term condition.​
