PROACTIVE CASE FINDINGS

One Health Lewisham are supportive of the National NHS @home initiative and are onboard with the opportunity to realise the benefits attached to a successful virtual care service. Proactive identification of patients who are deemed at a high risk of hospital attendance based on their current condition, or on past behaviour can be identified and onboarded to the service through this route. A referral will still be required to be onboarded to the service, but the proactive process ensures we are identifying early warning signs of a potential hospital attendance and can stage an intervention to prevent that occurrence as well as offering educational advice to the patient around self-care and how they can receive the right care at the right time in future.
