SOCIAL PRESCRIBING SELF-REFERRAL

Social Prescribing, often referred to as community referral, is a way of supporting your health and wellbeing by connecting you to events taking place in your local community. This can include a variety of activities such as exercise classes and social gatherings, particularly for adults who may feel isolated or require extra support in routine activities.

The objective of social prescribing is to address your wider emotional, physical, and social needs.
